Application to the program should take place relatively early in your degree program to ensure appropriate curriculum planning. Social Work students must submit the application by January 15 of the year preceding their concentration practicum. Advanced Standing students must submit the application by August 30.
Applying to MFT Certificate Program
Prior to submitting your application:
- Review the CMFT Program Handbook.
- Discuss your interest in the interdisciplinary certificate program in Marriage and Family Therapy with your regular academic advisor in your department.
- Identify an appropriate practicum site and discuss the opportunities for placement with your advisor, your Unit Coordinator, and the director of your graduate program (as applicable).
- In consultation with your advisor and using the Bulletin, plan your curriculum to meet your degree requirements and the certificate program requirements. The application asks you to indicate a detailed, proposed plan for how you will complete the certificate course requirements within your time to graduation.
When submitted, your application will be sent to the CMFT Program Coordinator. Please allow 2 weeks for a response. If admitted, you will be added to the MFT Certificate email list and receive important MFT certificate information, timelines, and course information.
Certificate Completion Forms
If you plan to graduate in May 2024, you must submit the MFT Completion Form and the Practicum/Internship Completion Form with all required signatures. To certify the completion of the MFT Certificate, the following tasks must be done by April 30th. Please read the following instructions in entirety prior to completing the forms:
- Complete the online MFT Certificate Completion Form here. Before you fill out the form, please read the following:
- This form requires you to indicate 1) each course you took to fulfill the certificate requirements 2) the semester the course was taken and 3) the grade received for the course. Be prepared to answer these questions before completing the form.
- Within the Completion Form, you will be required to upload a signed copy of the Practicum/Internship Completion Form prior to submission. This form must be signed by BOTH your site supervisor and your field liaison.
- Once you submit the Completion Form, you will see a prompt to 'Print or get PDF of answers.' Download and email a PDF of your form responses to your advisor. Your advisor should review the form and confirm that you have completed the coursework indicated on the form with their signature.
- Once your advisor has reviewed and signed the Completion Form, send the form to your CMFT Unit Coordinator for their final review. The Unit Coordinator should also sign the document on the last page.
